CLUB HOLLYWOOD PROGRAM GUIDELINES

1. Club Hollywood (CH) cardholders must be 21 years of age or older to participate in the program. CH members may have only one (1) account and may not play on or earn benefits from any other account.
2. No casino play or fee is required to obtain a CH card. When signing up for a card, you must present a valid and unexpired government-issued form or photo identification acceptable to Hollywood Casino Lawrenceburg (HCL).
3. It is the player’s responsibility to ensure that your CH Player’s Card has been properly inserted into the slot machine card reader. If the card is properly inserted, the display will welcome you and display a point countdown. For table play rating simply present your card to the dealer at any gaming table. HCL is not responsible for any unrecorded slot machine play due to an improperly inserted player’s card or any unrecorded table play due to failure of patron to give the dealer your player’s card.
4. CH members earn points when playing with their CH card inserted in the slot machine or by handing the card to the dealer before playing table games. Points are earned at variable rates depending on game type, game denomination, level of play, average bet or time played. CH members earn points that can be redeemed only at HCL.
5. CH has three tier levels of participation—Club Hollywood, Celebrity and Producer. Club member tier levels are evaluated semiannually, on June 30 and December 31, and may be adjusted based on their play for the previous six-month period. HCL management reserves the right to upgrade or downgrade any CH member at its sole discretion at any time without prior notice. Membership benefits will automatically expire on cards showing no activity for a 6-month period.
6. As a member of CH, you will receive offers and rewards available to members only. CH members give HCL express permission to send mail, email and other solicitations to the addresses provided, as well as telephone solicitations if your telephone number has been provided. Members may request at time of card activation, or subsequently in person, that they do not want to receive mailings or other information. CH members are solely responsible for ensuring that their address, email address and telephone number is on file and correct with CH.
7. HCL is not responsible for lost, stolen or damaged cards, coupons, vouchers or lost/misdirected mail or any other form of communication.
8. CH program benefits are not transferable and not valid in conjunction with any other promotions, except at the determination of HCL. Limit one (1) account per person.
9. HCL reserves the right to provide a CH member with a temporary upgrade in status. Conversely, HCL also reserves the right to reduce a member’s status, revoke specific member privileges and/or a member’s card altogether for actions that are deemed inappropriate, abusive or in violation of the rules and/or regulations of CH, HCL or the IGC.
10. Employees of HCL are not permitted to participate in this program. Employees of affiliates of Hollywood Casino may not be eligible to participate in all aspects of the program.
11. HCL reserves the right to refuse complimentary redemptions or rewards if it is determined that the true cardholder is not present or that coupons or other documents have been altered or tampered with in any way. Valid photo identification is required at the time of redemption of points for any reward.
12. HCL reserves the right to purge point balances from any player accounts without advance notice to the player.
13. HCL reserves the right to modify, change or cancel this program at any time without prior written notice.
14. For the new program that begins on June 26, 2009, card status will be determined by rated play for the 6-month period ending December 31, 2009. Moving forward, all CH members will be evaluated on a fixed 6-month period unless stated otherwise. The points earned in the evaluation period will determine card status.
